Guidance Notes for Team Nomination Form:

Please note:  To be nominated for an Excellence Award you must work within Wales 

Nominator reason for nomination
This section should be clear and precise. 250 words is a maximum and the nominator should not feel obliged to use the entire word count. Ideally the statement should be accurate and succinct. Data may be included if deemed relevant at this stage, any graphs or tables included here will not form part of the overall word count.

Lead Name:
This person should be the main team leader and the point of contact should the review panel require any clarification relating to the nomination. Joint lead names are permissible and designations of the lead/s would be beneficial.

Names of other team members:
Please ensure that all team members are included in the nomination. Designations would also be useful where possible. Please ensure that where teams involve patients and /or carers that these individuals are in agreement with their names being included in the nomination.

Work Address:
For the team nomination this address should be the main base for the team, if the team does not have a specific base then please use the address for the main team leader/point of contact.

Impact on local or national stroke services (Wales):
Impact should be identifiable either by qualitative or quantitative data. Please include information relating to metrics used and the collection of data. Inclusion of graphs, tables or separate testimonials will not be included in the word count. Data should be relevant and where possible any comparative data that supports the impact should be included.

Benefits to patients:
Any stated benefits must be identifiable. Testimonials from patients or carers may be included and will not form part of the overall word count. Please ensure that individuals providing testimonials are fully aware of the intended use of their contribution and that they are in agreement that the testimonial can be used for the purposes of the Award nomination.

Nominee name:
This should be the individual’s full name or working professional full name, with permission of the nominee it may include other names by which the individual is well known, please ensure that such inclusions are with the permission of the nominee as the name provided will appear on the award if successful.

Work Address:
For the individual nomination this address should be the main place of work for that individual. If the individual does not have a specific base then please use the address for a main point of contact.
